ApacheNetworks VOIP Disclaimer and Information
Internet Connection
Voice over Internet Protocol (VOIP) requires a high speed Internet Connection. It is the responsibility of the
Customer to maintain a high speed Internet Connection in order to have VOIP Services. ApacheNetworks cannot be held responsible for the quality
or outages of Internet Service provided by another Internet Service Provider.
911 Services
ApacheNetworks provides E911 services on VOIP lines, if you move your ATA, the number will always be tied to the physical address given at the time of installation.
E911 information can be updated if you move to another area, but can take up to 2 (two) weeks before it is updated in the E911 system.
Uninterrupted Power Source (UPS)
Apache Networks recommends the use of a UPS on any VoIP equipment. If a power outage should occur, service can continue for a brief p
eriod during the outage. In addition, the UPS will ensure the equipment (ATA) will still function after any power surges or power spikes.
If you subscribe to Apache Networks' Equipment Protection Plan, your ATA is warranteed for the duration of your agreement.
Analog Telephone Adapter (ATA)
An Analog Telephone Adapter (ATA) will be issued to all new Apache Networks Customers. The ATA will remain the property of Apache Networks.
The ATA charge of eighty ($80.00) will be waived when a one-year (1) contract is signed. In the event the one-year (1) contract is terminated
by either party the eighty dollars ($80.00) fee will be billed to the customer until the ATA is returned using the stamped address label
enclosed in the original ATA box. Customers on a month-to-month plan have the option to either purchase the ATA or rent it for $6.00/month. However,
the ATA must be returned upon termination of a month-to-month agreement using the stamped address label enclosed in the original ATA box with all of
it's original contents including documentation. Apache Networks will warrant the equipment for as long as a customer rents the equipment or subscribes
to our Equipment Protection Plan.
Early Termination on Installation Agreements
Customers who have signed up with one (1) year agreement will be charged an early termination fee upon termination of services whether it be
at the customer's request or by Apache Networks. The termination fee becomes due and payable immediately upon termination and will be billed
directly to the customer as well as any unbilled charges and the remainder left on the contract.
Payment & Collection
All payments are due before the 16th of every month. If payment is not received by the end of the month, services will be discontinued
and a disconnection fee will be applied to the account. If your service is disconnected, you will remain fully liable for all charges pursuant to this
agreement and any and all costs we incur to collect such amounts, including, without limitations, collection fees and attorneys' fees.
